The most basic element is size. The bedspread must fit the bed. The standard bed sizes are twin, full, queen and king, going from smallest to largest. You may also see California king, which is even larger than a standard king. Full is sometimes referred to as double. There are also sizes for younger children, and totally custom designs. Clearly if you try to put a queen size bedspread on a king size bed, there is going to be a shortfall somewhere.
Some bedspreads have down fillings to keep them extra warm. A heavy bedspread for the night can be used to save energy on the heating bill if you lower the thermostat every night. Real down comforters reflect body heat back to you so you can stay warm. They also absorb sweat if they get too hot during the night. The body cool during the night, so might be useful to have a bedspread that keeps you warm even on chilly mornings.
