The most basic element is size. The bedspread must fit the bed. The standard bed sizes are twin, full, queen and king, going from smallest to largest. You may also see California king, which is even larger than a standard king. Full is sometimes referred to as double. There are also sizes for younger children, and totally custom designs. Clearly if you try to put a queen size bedspread on a king size bed, there is going to be a shortfall somewhere.
The comforters and the bedspreads are often thought to be similar bed accessories - however, they are not. The bedspreads are typically larger than the comforters (even if they are for the mattress of the same size). This is because the bedspreads are intended to cover the complete bed, but the comforters' job is different. So while shopping for the two of these you shouldn't expect that the comforter will accomplish the job of the bedspread too. In fact some of the beds look very silly without the bedspread and with just the comforter!
